Patient Services Coordinator, LPN Residential Home Health is seeking a Full Time, Patient Services Coordinator!

SUMMARY: The Patient Services Coordinator is responsible for scheduling the home visits for the field staff and assisting with the coordination of patient care. In addition, the patient services coordinator will accumulate admissions/intake information and communicate with physicians about orders and patient progress information.

ESSENTIAL tasks AND RESPONSIBILITIES include the following. Other tasks may be assigned as necessary.
Compile the everyday schedules of clinical staff.
   » With the direction of the, adjunct Clinical Director assigns POD clinicians to patients.
   » Works with professional clinicians in developing the plan for patient frequency of visits that are clinically appropriate for the patient.
   » Assists POD personnel in care coordination of patient/client services. Serves as a liaison between the organization, referral sources, patients/clients and POD personnel.
   » Assists with management of lab results, adding them to the chart and dispersing the results to clinical staff as well as doctors.
   » Communicates with CFSS when anything is holding up scheduling.
   » Reschedules all visits that have been sent back from clinicians.
   » Other tasks as requested by Management.
   » Possible Weekend Rotation.

   » Proficient in Microsoft Office suite.
   » Able to communicate patient needs effectively to clinical staff.
   » Excellent telephone and customer service expertise.
   » Able to multi-task.
   » Able to problem solve.
   » Able to function as an integral part of the interdisciplinary team.
   » Able to work in a fast-paced environment.EDUCATION and/or practice:
   » Be a LPN/LVN licensed in the state in which he/she practices
   » At least one year of clinical practice in a hospital or nursing home setting
   » At least one-year of scheduling practice preferred.
   » Minimum of two years general office practice, with one (1) of those years having been in data entry or word processing functions.
   » Previous Home Health practice preferred
